The short answer

Apply online on the Swavlamban (UDID) portal, then attend the medical board at the government hospital you are assigned to; the e-certificate and UDID card follow once the assessment is approved. A disability of 40% or more is a benchmark disability under the RPwD Act, 2016, which brings 4% reservation in government posts and 5% in higher education seats. If the certificate is refused, Section 59 gives a right of appeal; denial of rights can be taken to the State Commissioner for Persons with Disabilities, Odisha.

A disability certificate in Odisha is now issued as a UDID (Unique Disability ID) certificate and card through the Government of India's Swavlamban portal, after an assessment by a government medical board. It is the document that unlocks most rights under the Rights of Persons with Disabilities Act, 2016 (RPwD Act): reservation in jobs and education, disability pension, travel concessions and other schemes. This guide explains how to apply, what happens at the medical board, and what to do if the certificate is refused or a right is denied.

Who can get a disability certificate

The RPwD Act recognises 21 specified disabilities, including locomotor disability, visual impairment and low vision, hearing impairment, speech and language disability, intellectual disability, specific learning disabilities, autism spectrum disorder, mental illness, cerebral palsy, muscular dystrophy, multiple sclerosis, dwarfism, acid attack victims, leprosy-cured persons, thalassaemia, haemophilia, sickle cell disease, chronic neurological conditions, Parkinson's disease, and multiple disabilities.

The extent of disability is assessed under guidelines notified by the Central Government (Section 56). A person with 40% or more of a specified disability is a person with benchmark disability, and most reservations and schemes are tied to that threshold. Some benefits need a higher percentage; in Odisha, for example, the enhanced pension is linked to 80% or more.

How to apply for the UDID card in Odisha

  1. Apply online on the Swavlamban (UDID) portal. Choose "Apply for Disability Certificate and UDID Card", select Odisha and your district, and fill in personal details, address and the type of disability.
  2. Upload a recent photograph, signature or thumb impression, and proof of identity and address (Aadhaar is commonly used). If you already hold an older paper disability certificate, upload it; it can be migrated to UDID.
  3. Note the enrolment number. Help is available at the District Social Security Office (DSSO) and at Block or urban local body offices, and the department holds camps for people who cannot travel.
  4. Attend the medical board on the date given. In Odisha, assessments have been done at District Headquarters Hospitals and authorised medical colleges; in July 2025 the Social Security and Empowerment of Persons with Disabilities (SSEPD) Department proposed extending this to Sub-Divisional Hospitals to clear backlogs. Ask the DSSO or the hospital which centre handles your type of disability.
  5. Collect the certificate. Once the board's assessment is approved, the e-certificate can be downloaded from the portal and the UDID card is sent by post. You can track status on the portal using the enrolment number.

A certificate issued under Section 58 is valid across India. The certificate states whether the disability is permanent or for a limited period; temporary certificates must be renewed before they expire.

What the medical board looks at

Take all previous medical records, test reports (for example audiometry for hearing, IQ assessment for intellectual disability, or imaging for locomotor disability), prescriptions and any earlier certificate. For children, school records and developmental assessments help. If the board needs tests it cannot do on the same day, you may be given another date. The board either issues the certificate with the percentage assessed or tells you in writing that you do not have a specified disability (Section 58(2)).

If the certificate is refused or the percentage looks wrong

Section 59 of the RPwD Act gives a right of appeal against the certifying authority's decision to the appellate authority the State Government designates, within the time and in the manner the State prescribes. Ask the DSSO or the Chief District Medical and Public Health Officer (CDM&PHO) for the current appellate authority and time limit in your district, and file in writing with the assessment papers and fresh medical opinions. If a certificate is not issued within a reasonable time, an RTI application asking for the file status, or a grievance to the State Commissioner (below), often moves it.

Disability pension in Odisha

Persons with disabilities in Odisha are covered under the state's Madhu Babu Pension Yojana (MBPY) and the central National Social Assistance Programme. In January 2025 the state cabinet raised the monthly pension to ₹3,500, initially for beneficiaries aged 80 or above and persons with 80% or more disability. Others continue at the lower rate; check the amount currently payable with your Block or urban local body office. Applications are made at the Block office (rural areas) or the urban local body office, and sanctioned through the District Social Security Office; the UDID certificate is the proof of disability.

The SSEPD Department also runs scholarships, assistive devices, marriage incentive and other schemes for persons with disabilities. The current list and forms are on the SSEPD Department website.

Complaints when a right is denied

  • Grievance Redressal Officer: every government establishment must appoint one under Section 23. Complain to that officer first, in writing.
  • State Commissioner for Persons with Disabilities, Odisha: under Sections 79 and 80, the State Commissioner can inquire into complaints about deprivation of rights and recommend corrective action to the authority concerned. The office functions under the SSEPD Department in Bhubaneswar.
  • Chief Commissioner for Persons with Disabilities (New Delhi) for matters involving central government establishments, such as railways, banks or central PSUs.
  • Special Court: under Section 84, each district has a Sessions Court designated as a Special Court for offences under the Act. Section 92 punishes insulting, humiliating, assaulting or denying food or fluids to a person with disability, among other acts, with imprisonment of six months to five years and fine. A police complaint can be lodged in the ordinary way (see how to file an FIR).
  • Orissa High Court: where a public authority refuses a reserved post, admission or certificate in breach of the Act, a writ petition lies.

Persons with disabilities are entitled to free legal services under the Legal Services Authorities Act, 1987; see free legal aid in Odisha.

Frequently asked questions

How do I apply for a UDID card in Odisha?

Apply on swavlambancard.gov.in, choose Odisha and your district, upload a photo and ID, and attend the medical board on the date given. The District Social Security Office and Block or ULB offices help with applications. Track the status with your enrolment number.

Is the UDID card the same as a disability certificate?

Yes. In Odisha the UDID certificate generated through the portal serves as the disability certificate for schemes, and a certificate issued under Section 58 of the RPwD Act is valid throughout India.

What is benchmark disability?

A person with not less than 40% of a specified disability, as certified by the certifying authority. Reservation in government jobs and higher education, and many schemes, are available only at or above this level.

How much is the disability pension in Odisha?

Under the Madhu Babu Pension Yojana, the pension was raised to ₹3,500 a month in 2025, initially for persons with 80% or more disability (and people aged 80 and above). Others get the lower rate; confirm the current amount at your Block or ULB office.

What can I do if my disability certificate shows a lower percentage?

Section 59 of the RPwD Act allows an appeal to the appellate authority designated by the State. Ask the DSSO or CDM&PHO for the authority and time limit, and file with your medical records and any fresh opinion from a doctor in the relevant field.

Where can I complain if a government office ignores disability reservation?

Complain first to the office's Grievance Redressal Officer, then to the State Commissioner for Persons with Disabilities, Odisha. A writ petition before the Orissa High Court is available when a public authority acts contrary to the Act.
